Individualized Education Program (IEP)

The Individualized Education Program (IEP) is a written document that’s developed for each public school child who is eligible for special education. The IEP is created through a team effort and reviewed at least once a year.

  • Use this IEP Checklist as a tool to make sure the IEP team fully addresses your child’s Physical Education placement and services (Developed by Scott McNamara, Lauren Lieberman, Brad Weiner, and Garth Tymeson)

  • This document provides an example of an evaluation report in order to meet the requirements of Adapted Physical Education by the Minnesota Department of education
