PK-12 Physical Education and Adapted Physical Education Practitioner Application - First Time Certification

All portions of the application must be completed online at one time so please be sure to have all of your information and needed documentation available before you begin the application. The application cannot be saved nor returned at a later date. All portions of the application need to be submitted to and reviewed by the APENS Committee before an examination period will be scheduled. A checklist is provided to help you gather the documents needed and to guide you as you complete the application. Consider printing it for use.

    • ❏    Provide evidence of professional preparation.
      • ❏    Provide evidence of completing eligible coursework in adapted physical education.
      • ❏     Send relevant college transcripts to APENS
      • ❏    Upload PDF copies of any or all current licenses or certificates you possess to teach physical education in your state.
    • ❏    Provide evidence of 200+ hours of teaching physical education to students with disabilities.

The NCPEID reserves the right to reject any application that does not meet eligibility criteria as documented in this application.
